The Forkan will ask for Restorative Justice for Dinar Wahyu and a possible return to normal for ATG with the recovery of your deposits from V3. The more people registered, the easier it will be to approve. They want all members to respond to the Google Form: those who have recovered everything as well as those who have been harmed.

The documents are to be uploaded on the Google Form of the Forkan. This should include screenshots with proof of account ownership, proof of account transfer - highlight deposits and withdrawals and power of attorney (next document).

1 - Last name on your identity card

2 - Identification number (NIK KTP)
This is your passport or identity card number

3 - Postal address identical to your identity document

4 - Current home address

5 - Photo of your ID (to be uploaded)

6 - Email used when registering on ATG

7 - Mobile number or WhatsApp

8 - Total loss (Deposits - Withdrawals = loss).
Do not forget to indicate the currency (USD)

9 - MT4 account number for ATG / ATC
Example: ATG: 123456, ATC: 543212

10 - Proof of ownership of your MT4 accounts (to be uploaded)
Example: mobile phone, email, screenshot, website homepage, etc.

11 - Optional document. Download the crypto statement showing the transactions in PDF format (to be uploaded) (highlight all transactions related to deposits or withdrawals).

12 - Download Power of Attorney (to upload, see below) 🔽
